Suspicion of Abuse
In any case of suspected abuse, the healthcare provider should be prepared to document and describe the victim's:
- name
- address or location
- approximate age
- race and sex
- Indications of harm or injury
- Alleged perpetrator contact information
- Relationship of the alleged perpetrator
If the relationship is unknown, a report will still be taken if other reporting criteria are met.
