According to the Committee on Quality of Healthcare in America, the majority of quality problems and medical errors occur because of fundamental shortcomings in the ways care is organized (IOM, 2001, p. 5), not individual error or negligence (Kohn, Corrigan, & Donaldson, 2000).

An adverse event is any injury caused by medical management rather than the underlying condition of the patient.

An adverse event attributable to error is considered a preventable adverse event (Kohn, 2000, p. 28).
